Policy on Cancellations & Late Arrival
Because of our small business size and high demand for appointments, short notice cancellation appointments failed attendance or late arrival (of more than ten minutes) disrupts service delivery to our clients and the operation of our employees.
We are pleased to announce The Cancellation Policy at Optimum Smile Dental.
1- Patients are NOT generally required to pre-pay for dental consultations or treatments do not excess 30 minutes in chair. However, a $50 deposit is required for each 45 minutes or longer appointment and it will be attributed to future transactions.
2-If patients cancel or reschedule an appointment or do not attend, offer less than 24- hour notice:
+ For the first occurrence, we understand that unexpected situations can happen (extreme weather, emergencies), and we will NOT request a late cancellation fee.
+ For second and subsequent occurrences, late cancellation fees will be applied and we often request a deposit prior to booking appointments. The deposit will be requested to reserve a further appointment and be attributed to future transactions.
3- Cancellation Fees are non-refundable.
PAYMENT POLICY
All dental treatment fees are required to be paid on the day of treatments unless payment plans in arrangement.
With some major dental treatments, we reserve the rights to request full payments prior to the completion of the treatments.
*With Professional Teeth Whitening, because of the duration and preparation costs of the therapy, we request a booking deposit of 50% of the treatment fee at the time of appointment confirmation. The remaining fee will be charged on the day of treatment.
